Camper Health Form and Pre-Camp Medical Clearance

Sandy Hill uses an online health information system that is accessed via your Camper's Home Page. The only items that are not completed online are the Pre-Camp Medical Clearance page and any Medication Administration Authorization Forms (see the next section for MAAs).
Medication Administration Authorization (MAA) Forms
New in 2018, there are now multiple versions of this form. Since some doctors office charge per page for completed forms, we have created multi-medication versions to help. Note that all medications on any given form must be approved by the same prescriber.

Asthma Medication Forms
These forms must be completed and signed by a parent/guardian and
an authorized prescriber for a medication to be administered
at camp. This includes prescription and over-the-counter including
vitamins, herbal medicines, and homeopathics.

Please have your physician complete and sign this form for all items relating to asthma including your action plan and all asthma-related medications. Please note that this asthma form, when fully completed and signed also meets the state requirements for a Medication Administration Authorization (MAA) form. This means that you do not need a separate MAA for any medications listed and approved on this asthmas form.
Religious or Medical Exemption to Immunizations

Please note that per Maryland regulations, in the event of an outbreak
at camp, any campers who are not current on their immunizations are
required to leave camp.
